Dear Patients,

I will be retiring and closing my practice on May 1st, 2011. It has been my privilege to care for you over the years. My staff and I value your trust and your continued well-being is our priority. To that end, I have asked Dr. Erik Miles to provide continued care to our current patients that may need immediate attention or follow-up. Dr. Miles is an experienced, highly skilled and compassionate Plastic Surgeon whom I have worked with personally and trust to provide you excellent care. Dr. Miles is Board Certified by the American Board of Plastic Surgery. Alternatively, you can contact the North Carolina Medical Society at
919-833-3836 or your health insurance carrier for a referral.

For safe keeping and continuity of care, Dr. Miles has also agreed to be the Legal Guardian of all retained medical records from my practice. Should you need copies or transfer of your medical records, please notify us prior to May 1st, 2011. After the close of my practice, signed requests for copies of medical records can be directed to Dr. Miles' office. Dr. Miles may be reached at 704-896-5556.
